Some men find that one or both breasts may be abnormally large. Known as gynaecomastia, this condition can result from a hormone imbalance or from being very overweight, which can increase levels of oestrogen and cause breast tissue to grow. Breast reduction surgery is an option for men with this condition. It may also be used to tighten the breast area after dramatic weight loss that has caused the skin to sag. However, it is not suitable for men whose large breasts are simply excess fat from being overweight.

What does it involve?

Breast reduction surgery is usually carried out under general anaesthetic. There are several techniques the surgeon could use, but generally the operation involves:

making a cut (incision) around the nipple

using liposuction to suck out excess fatty tissue

extending the cuts and repositioning the nipples if there is a lot of tissue to remove

The operation takes about 90 minutes. You usually need to stay in hospital overnight.

Recovery

It can take a few weeks to fully recover from breast reduction surgery. You may need to take a few days off work. It can take up to six months to see the full results. You will need to wear an elastic garment day and night for a few weeks after the operation. The length of time you need to keep the dressings on depends on how quickly your wounds heal. After one or two weeks, your stitches will either dissolve or be removed. You should avoid stretching, strenuous exercise and heavy lifting for up to six weeks after the operation. You can drive again when it’s no longer painful to wear a seatbelt, which may be several weeks.

As a general rule, every 125-150cc of implant volume equals an increase in a single cup size. However, every patient’s body is different, you can set obtainable and healthy expectations together. It is helpful for you to bring in a bra the size you would like to be to help in finding the right sized implant for you.

Will my nipple sensation or feeling change?

Most women will have loss of some feeling or sensation following breast augmentation dubai surgery. For most of them, this lasts 6-12 months before returning to normal. 15% of women, however, have permanent alterations in nipple sensation.

How long until I can exercise?

It is recommended that patients begin walking immediately after surgery. However, women should not perform any intense physical exercise for six weeks following the operation. Physical exercise including weight lifting, biking, jogging, and other forms of intense activities may cause implants to shift position or cause wound healing problems that may alter the appearance of the breasts after surgery.

When can I return to work?

How much pain can I expect?

Can I breast feed after augmentation?

Yes. Placement of the implant below the breast tissue, as in subglandular placement, does not affect the ability of the breast to produce milk. Similarly, submuscular placement, or implant placement below one of the chest muscles preserves proper breast functioning. With the peri-areolar incision, an increased risk of breast-feeding problems may exist. In one study, 7/8 patients reported problems with breast-feeding following peri-areolar breast augmentation surgery. For women who choose the infra-mammary or trans-axillary incision, (incisions under the breast and through the armpit), breastfeeding is usually not a problem. Be sure to discuss your breast feeding needs with your surgeon during your consultation.

Will I need additional surgeries?

Sometimes additional surgeries may be required for breast augmentation dubai patients. To reach optimal symmetry, you may desire follow up breast lifts or other supporting procedures. Additional surgeries may be needed if you face any complications during your augmentation, like implant rupture or capsular contracture. There are a variety of abdominoplasty, or tummy tuck, procedures available, and operations can be tailored to suit a patient’s wishes and needs. Generally speaking, however, tummy tucks are most commonly carried out to remove excess skin, unwanted scars, stretch marks and fat, and to tighten the stomach muscles. The ideal patient is someone whose weight is normal, particularly those who have been overweight and have lost the excess weight. Abdominoplasty is not an operation for people who are overweight, or as an attempt at losing weight. In appropriate patients abdominoplasty can have a very marked effect and is associated with high patient satisfaction.

Full abdominoplasty
For patients who have significant skin laxity, excess fat and separation of the muscles, a classic tummy tuck is the most common procedure. Performed under general anaesthetic, this operation can require patients to be in hospital for two or three days.

During the operation, an incision is made from hip to hip and around the umbilicus. The excess skin and fat is excised from the umbilicus to just above the pubic hair. The muscles above and below the umbilicus are tightened. The skin is then sewn up to give a circular scar around the umbilicus and a long scar across the lower abdomen. Although this operation leaves a large scar, it does provide the greatest improvement in abdominal shape. Patients who are thinking about becoming pregnant should not undergo this procedure, and should wait until they are sure they are not having any more children.

Mini abdominoplasty
For patients with only a small amount of excess skin a lesser abdominoplasty might be appropriate. A general anaesthetic is still needed.

During the operating, a wedge of skin and fat is excised from the lower tummy leaving a horizontal scar above the pubic hair. Sometimes the muscles will also be tightened. No scar is left around the umbilicus, which may be stretched slightly to become a different shape. A mini abdominoplasty will give a smaller effect than a full abdominoplasty.

Liposuction
Liposuction can sometimes be done at the same time as the abdominoplasty. However, liposuction can be recommended instead of an abdominoplasty for younger patients who have good quality, elastic skin and whose main problem is a localised excess of fat.

A general anaesthetic is most commonly used, but treatment can often be as a day case. During the procedure, small incisions are made, excess fat is removed and the wounds are then closed. Some degree of skin tightening or shrinkage can be expected as well, but the results of liposuction can sometimes be unpredictable. Liposuction alone will have less effect than abdominoplasty.

Are they safe?

What are the risks?

Risk is minimal compared to invasive surgical treatments which require cosmetic / plastic surgery, however it’s not uncommon to experience minor side effects as dermal fillers are injection based treatment and pierce the facial skin direct.

What are the side effects?

A side effect can be very light bruising. Lasting up to three days, medically this bruising process simply occurs as healthy bloodstream works to instantly treat the broken skin tissue and live wound – the process induced via the needle injecting point. This is temporary and lasts normally anything from 3 days up to a week. It should be noted that other interference issues may also provoke bruising including medications, lifestyle and alcohol. Lifestyle factors – Skin aging process is influenced by both intrinsic and extrinsic factors. While degradation of collagen and elastin fibers as well as the reduced quality of the body’s own hyaluronic acid are mainly genetically encoded, there are extrinsic or lifestyle factors which we all can control to a certain degree to reduce skin ageing. Smoking and drinking, excessive sun exposure and stress, can all speed-up the biological clock. However, of all these aging factors sun damage by UVR poses the greatest threat. Restylane products cannot turn back the passage of time. However, they can restore skin hydrobalance and improve the skin structure and elasticity, as well as fill out wrinkles and folds and restore facial volume.

Facial aging – Facial aging is due to changes in several types of tissue, including skin, fat, muscle, and bone. Changes in any one of these structural layers affects the other layers.

Skin – As skin ages, the middle layer of skin (dermis) thins due to collagen loss. The skin’s ability to retain elasticity (from elastin) and moisture (from hyaluronic acid) also reduces with age. Due to this loss of elastin and hyaluronic acid, the skin becomes:

Fat – In the face, fat is organized into fat pads. As we age, the fat is lost or redistributed, and the fat pads shift relative to each other. Over time, bony features, lines, and wrinkles become more visible.

Bone – There is a significant loss of facial bone with age. Aging of the craniofacial skeleton may be due to changes in the relative dynamics of bone expansion and bone resorption. Bone recession leads to thinner bone, removing structural support for the overlying tissues. This loss of support contributes to sagging and droopiness and noticeable changes in the other layers of overlying soft tissue and skin.

Types of Fillers & How They Work

Unlike botulinum toxin injectables—such as Botox, Dysport or Xeomin – which impact the way muscles move in your upper face, dermal fillers are meant to “take up space” just under the skin to fill in lines and restore volume lost to aging.

HYALURONIC ACID DERMAL FILLERS

Hyaluronic acid dermal fillers are well-loved for their long-lasting, natural-looking results. Here’s the lowdown on hyaluronic acid fillers like the Juvéderm family of fillers, Belotero Balance and Restylane Lyft.

For the science buffs: hyaluronic acid (HA) is a glycosaminoglycan found in epithelial, neural, and connective tissues in humans. For the rest of us: HA is a carbohydrate that naturally occurs in the body. Because it attracts water, HA helps keep skin lubricated, supple, and youthful-looking. The dermis (deepest layer of skin) is composed of hyaluronic acid and collagen—and as we age, our reserves of both are naturally depleted.

The reason HA makes such a powerful ingredient in dermal fillers is the way it interacts with the moisture in your skin. In gel form, HA will swell when combined with water, which ultimately smooths and fills lines and wrinkles while restoring natural-looking volume. The HA used in dermal fillers is either synthesized using bacteria or derived from animals, such as rooster comb. Because it mimics the HA already in our bodies, there is very low risk of adverse reaction.

Thanks to advancements in our understanding of the aging process, how hyaluronic acid works, and the quality of available dermal fillers, we are able to provide more consistent, longer-lasting results. Often, the hyaluronic acid in dermal fillers has gone through a crosslinking process, where the molecules have been chemically bonded to help them last in the body for longer periods of time.

Not all hyaluronic acid fillers are the same, and certain products are better-suited to addressing certain concerns. The consistency, size of the hyaluronic acid molecule, and how the molecules are bonded all impact where a filler works best and how long results will last.

NON-HYALURONIC ACID FILLERS

While hyaluronic acid fillers may be popular, they’re not your only option—and sometimes an alternate product will provide you with better results. Non-hyaluronic acid fillers are composed of a variety of ingredients ranging from collagen to mineral compounds. Often, these products provide a two-fold benefit: they physically fill in lines and plump up the skin in addition to helping your body make more collagen for a fuller, healthier look.

While you can likely expect nice looking results from any of the products, it’s important to be open with your provider and provide clear information about your expectations and any allergies you may have. It’s also worth your while to ask what can be done if you do not like your results – unlike hyaluronic acid fillers, the above fillers cannot be “undone” by anything other than time.

POPULAR DERMAL FILLER BRANDS

The Juvéderm family of fillers include multiple formulations specifically designed for treating different areas of the face and addressing a variety of concerns. All Juvéderm products contain lidocaine to help ensure treatment is as comfortable as possible.

  • Juvéderm XC instantly softens the appearance of moderate to severe creases on the mid- and lower-face for up to a year.
  • Juvéderm VOLLURE XC is a firmer, crosslinked gel that is ideal for smoothing deeper wrinkles around the nose and mouth for over a year. It uses Juvéderm’s patented VYCROSS technology (a particular ratio of large and small HA molecules) to create a very smooth result that is still durable enough to handle the movements of the lower face beautifully. VOLLURE is great for treating smile lines, marionette lines, and nasolabial folds.
  • Juvéderm VOLUMA XC is used to restore volume in all three areas of the cheeks and create a youthful contour in the midface. It provides a subtle lift, helping patients look noticeably younger, for up to two years.
  • Juvéderm VOLBELLA XC can subtly enhance lip volume, improve lip definition, and soften vertical lip lines. It has a lower concentration of HA in differently weighted particles to ensure it is soft enough to provide very natural results but durable enough to last at least a year.

Like Juvéderm products, Belotero Balance is formulated with hyaluronic acid and designed to fill in creases and correct age-related volume loss. What makes these two products different is their crosslinking, how cohesive the product is, and the firmness of the gel. Belotero is less viscous and thus able to spread to all the small areas of the dermis, which is why it is often recommended for treating “etched in” creases.

Restylane Lyft, on the other hand, is a denser hyaluronic acid gel with a higher molecular weight, meaning it’s a good option for treating deeper nasolabial folds, filling in severe lip lines, and adding volume to the cheeks.

Bellafill fills in lines and improves the appearance of scarring for up to five years. It contains a mixture of collagen gel and polymethyl methacrylate (PMMA) microspheres to improve wrinkles in two ways: it physically restores volume and plumps deep wrinkles and scars as the product rests under the skin and it provides an internal support structure to prompt your body’s natural collagen production. That means that while you’ll see see instant results, your skin will look and function better long-term and results will improve with time.

Radiesse contains calcium hydroxylapatite, which is a mineral compound found naturally in human bones and cartilage. When injected, it quickly acts to replenish lost volume while simultaneously prompting collagen production for lasting improvement. Radiesse can be mixed with lidocaine to reduce discomfort during injection. In addition to correcting facial wrinkles, Radiesse is often favored for hand rejuvenation.

Sculptra is an injectable poly-L-lactic acid that works by stimulating collagen production, restoring your skin’s inner structure, and increasing volume subtly and gradually over time. Sculptra is unique in its ability to provide improvement for at least two years after a treatment course of three to four injection sessions.

Changed the doctor last minute simply because he couldn’t do the surgery the way I wanted it to be done. I wanted the implants under the muscle not above so I found doctor vego who introduced me to third option which is Dual Plane. During my researches I read about this technique that is most used by doctors recently especially in Thailand but I didn’t think I would meet a doctor who would be able to do it for me. What I liked about My doctor is that he gave me all the options plus his own opinion and recommendations and left me the space to choose the incision, implant type and size, and implant placement, etc. He is surrounded by strong team including his assistant who was available 24 hrs to answer my questions and worries and planned everything for me as I only had two days between consultation and surgery. Anastasia doctor was super nice and nurses as well. Supported me and helped me with everything as I came by myself to the hospital to do the surgery and you can imagine how scared and lonely felt. Post surgery: no bleeding, no infection, very very light aching compared to other people’s experiences. Recovery was quick without any complications. After two weeks of free follow up I was able to travel for holiday and I resumed work exactly after 20 days considering that it’s a very physically demanding job yet everything went well.

Few years later I went through another plastic surgery (Otoplasty) to fix a defect in my ears shape. The results were amazing and far beyond my expectation, Why? Because it was him again who did it.
