BREAST AUGMENTATION: BREAST IMPLANT TYPE, SHAPE & TEXTURE

Cons: In the event of a rupture, the danger of silicone implants is that they do not reabsorb into the body. The so-called ‘silent rupture’ requires an MRI scan to diagnose. Additionally, inserting this type of implant requires a larger incision, which may factor into the decision-making process for some.

SALINE BREAST IMPLANTS

What: A silicone outer shell that is filled with saline (i.e. saltwater).

Pros: They can be preferentially filled with a range of fluid (saltwater), which allows better customization of size. Because they aren’t pre-filled, a smaller incision is needed. This allows them to be rolled (like a cigar) into a much smaller profile and inserted through a much smaller incision. If saline implants rupture, the saltwater is harmless and reabsorbed by the body.

Cons: Rupturing is a big concern for many breast implant patients, and, if a saline implant ruptures,  it will leave you “with what Dr. Vigo lovingly refers to as the ‘flat tire’ look.” The breast will have a visible deflated appearance that requires surgery to correct. Additionally, saline implants can have a wrinkled or rippled appearance under thinner skin.

Breast Implant Shape

Now that you know the difference between silicone and saline-filled implants, it’s time to talk about shape. There are really only two different shapes of implants: round and anatomic (a.k.a. teardrop). This impacts the profile of the breast once the implant is inserted.

ROUND BREAST IMPLANTS

What: Round implants are circular and have a uniform circumference.

Pros: In the event of a rotation, the round shape does not appear to have moved the same way a teardrop might. Round implants can be filled with silicone or saline and housed in a textured or smooth casing.

Cons: There are few notable cons to round-shaped breast implants. Depending on their aesthetic goals, some patients may express dissatisfaction based on individual preference.

TEARDROP BREAST IMPLANTS

What: Anatomic or teardrop breast implants have more volume at the bottom.

Pros: Because teardrop breast implants are tapered, they theoretically better mimic the shape of a natural breast. To minimize the risk of rotation and asymmetry, they have a textured exterior .

Cons: The early buzz of anatomic implants was that they would give a more natural result. The theory, however, did not bear out. Instead, the big disadvantage of the teardrop-shaped implants is their ability to rotate. In order to maintain the shape, teardrop implants can only be saline-filled.

Breast Implant Texture

As we started to discuss above, the texture of the implant also matters. Implant texture refers to how the outer shell feels (think: smooth or textured). There are pros and cons to both:

SMOOTH BREAST IMPLANTS

What: The exterior of the implant has no texture.

Pros: Without any texture to the shell, the implant may have a softer look. Without the texture to lock it in, smooth implants can move around more freely, which allows it to feel more natural.

Cons: This style can be more prone to inflammation around the implant and may not be the right fit depending on your medical history. Additionally, there is more of a chance of rotation, though it’s not usually very noticeable in round-shaped implants.

TEXTURED BREAST IMPLANTS

What: The shell has a rough exterior that is comparable to sandpaper.

Pros: Texturing is thought to potentially reduce the risk for capsular contracture, but it is also used when the implant is shaped — instead of round — to avoid rotation.

Cons: For the implant to have a textured exterior, it requires a thicker shell that may lead to a less natural-looking appearance. Additionally, you may remember the recall involving textured implants from a couple years ago. These implants have been tied to the risk for a very rare cancer called anaplastic large cell lymphoma (ALCL) — a form of Non-Hodgkin’s Lymphoma.

Profile

Not to be confused with size, the profile or dimension of the implant will dictate the protrusion of the breast. Given the same amount of volume, we have implants that are wider and flatter and we have implants that are narrower and more projected. This allows for even further customization of a specific look for your patients. A lower profile implant, for example, will only protrude a little, whereas the high profile option will appear fuller. While that’s the general rule, the exact outcome depends on the individual’s anatomy – meaning the size and shape of the chest wall and breast envelope.

Summary

Who is not a candidate for breast augmentation?

– You have breast cancer or an abnormal mammogram.

– You’re ill or battling an infection.

ABDOMINOPLASTY DUBAI: FULL, MINI & EXTENDED WHAT IS RIGHT FOR YOU?

A flat, sleek abdomen is a very common desire, and many people put a lot of effort into achieving it. Although there are some who can achieve it through diet and exercise, there are others for whom these will never be enough. Whether because of pregnancy, a major weight loss, or simply because of genetics, some people will never have a flat belly, even when they’re at a healthy weight and are fit.

If you’re one of the many who have struggled with this issue, you may have considered a tummy tuck. Medically known as abdominoplasty, it’s one of the five most common cosmetic surgical procedures performed. (Liposuction is not the same as a tummy tuck, although these can sometimes be performed together.) There are a few different options, and which one is right for you depends on your particular situation and your goals for your appearance. What are the advantages and disadvantages of the various options? How can you choose which type of tummy tuck is right for you?

FULL OR CLASSIC TUMMY TUCKS

Classic tummy tucks (which are also referred to as full tummy tucks) are utilized when individuals want to address the sections above and below their belly button, the full length of the abdominal wall. Patients who opt for a full tummy tuck often want to resolve general roundness and excess skin. With this procedure, one horizontal or u-shape incision, and possibly a second incision, are made above the pubic area and the belly button, respectively, to remove excess skin and tighten muscle. Full tummy tucks present the most natural-looking outcome and can include a much firmer abdomen.

MINI TUMMY TUCKS

Mini tummy tucks involve less extensive incisions compared to a full tummy tuck and are intended to improve only the lower belly, below the patient’s belly button. This makes the procedure ideal for people who want to rid their bellies of a “pooch”, tiny bulge or stretch marks in the lower abdominal region only. Plastic surgeons can tighten loose muscles through a single low horizontal incision (typically without an incision around the belly button), and remove excess lower abdominal skin, resulting in a firmer, smoother lower belly.

EXTENDED TUMMY TUCK

The extended tummy tuck is considered when the goals of the procedure are to address the abdomen while simultaneously targeting the flanks and hips. To perform this procedure, the surgeon extends the horizontal incision more laterally as well as often incorporating a longer, vertical incision, which enables the surgeon to remove excess skin from the upper and lower abdominal region as well as the flanks while tightening the abdominal wall. This procedure is often ideal for those patients who have had multiple pregnancies or have lost lots of weight, either through diet and exercise or through weight loss surgery.

When it comes to a tummy tuck, I frequently explain to patients that the procedure is not a single surgical step, but rather a series of elements that can be combined to create customized results.

Whereas liposuction focuses solely on removing unwanted fat cells from the body – such as those that create a noticeable bulge at the belly – a tummy tuck is intended to create sleeker contours by addressing any number of conditions that keep the abdomen from looking toned.

Excess fat

Of course, stubborn pockets of fat are an obvious culprit when it comes to a larger waistline. While liposuction is frequently performed as a standalone procedure, it can also be used as part of a tummy tuck to help flatten the area. For this element, just about everything is handled as it would be for a solo lipo procedure: Fat is loosened and suctioned out via a cannula. The difference is that while liposuction requires small incisions that are later sutured closed, the incision made for a tummy tuck allows the surgeon access.

Excess skin

While extra fat causes bulges, extra skin creates a sagging pouch that can hide even well-defined abdominal muscles that sit beneath. The amount of excess tissue can range from a little to a lot (sometimes referred to as an “apron”). For a tummy tuck, the surgeon essentially re-drapes the abdominal skin so that it sits flat and tight, then trims away the excess. The incision and subsequent sutures for this element are typically hidden lower on the torso, where they can be covered by a swimsuit bottom or underwear.

Lax muscles

Over time, belly muscles and their covering fascia can become loose. Whereas firm fascia holds everything in place – something like interlaced fingers keeping a water balloon from bulging outward – lax fascia allow the contents behind to bulge outward. Abdominal muscles can separate, giving the entire midsection a flabby look. During a tummy tuck, a surgeon can, if necessary, stitch these muscles and fascia back together to make everything tight and toned. In some cases, as with a mini-tummy tuck, this may be the primary element of the procedure. It is often chosen by moms who have a small bulge that sits below the belly button.

Belly button placement

Depending on the tummy tuck details, the surgeon may need to “relocate” the belly button, which involves creating a hole in the newly taut abdominal skin so the navel appears where it should to remain proportional with the rest of the body. This is called “umbilical translocation.” Following this, the body can be further contoured to encourage an hourglass shape by creating an “internal corset.” I have performed measurements that show narrowing of the waist and improvement of the waistline due to work at this stage.

When all of the steps are completed, the incision will be closed with either a special glue or absorbable sutures, such that the patient can take a shower the next day and there is no need to remove the sutures. Lasers can be used post-operatively to hasten the healing of scars. Also, use of Exparel injections numb the abdomen for three days, which can minimize pain for patients.

Since every patient has a unique body shape and size, as well as personal goals for an ultimate look, each tummy tuck is its own procedure. While there are commonalities between all such surgeries, no two will be exactly alike.

The most common form of tummy tuck involves a horizontal incision near the pubic hairline, so swimwear or undergarments can hide the resulting scar. If there is excess skin above the navel, an incision around the navel may be required. Sometimes, the surgery involves liposuction to remove pockets of excess fat from specific areas of the body. To reduce scar tissue and increase the speed of healing, I may utilize a harmonic scalpel that uses ultrasonic energy to perform parts of the tummy tuck surgery. Diastasis is a term used to describe the separation of the two-paired abdominal muscles that make up the front of the abdomen, which is often caused by multiple pregnancies. Sutures are used to tighten up the abdominal wall, resulting in a flatter stomach. Stronger, tighter abdomen muscles may also help reduce lower back pain.

A tummy tuck is not a shortcut to weight loss. But, it can be a powerfully transforming surgery and complements the impact that weight loss has on the abdominal profile. Many of our patients say they feel liberated after having a tummy tuck. They can wear a bikini again, or not wear a shirt or a top to cover up their loose stomach. They feel good about being outdoors running, swimming and golfing. It does a lot to boost a person’s confidence in their bodies and for many of my patients, it’s an incentive to maintain a healthy appearance for the rest of their lives. For some patients, tighter abdomen muscles may also help reduce lower back pain.

DETERMINE WHICH PROCEDURE IS RIGHT FOR YOU

Deciding which procedure is right for you will be contingent upon several factors and is best done in consultation with an experienced Plastic Surgeon. Some of them include your desired results, overall skin quality, body type and location of excess skin on the body. The amount of excess skin will also determine which procedure is most appropriate. Finally, any scarring attained from a previous cosmetic procedure or surgery can determine which kind of tuck the patient qualifies for.

RECOVERY

You’ll want to figure out a time frame for your recovery so you have ample time to heal and can take a break from certain aspects of your life. Make sure you make the proper arrangements and are fully prepared for your recovery period.

Your drains will be left in for a few days after the surgery. You’ll be shown how to take care of and empty the drains. You’ll likely need to take an antibiotic and an anticoagulant while your drains are in place.

You’ll wear an abdominal binder for about six weeks. This helps to avoid fluid build up and helps to support your abdomen.

While the recovery period is usually shorter for a mini-tummy tuck, you’ll still need to avoid strenuous activity for at least six weeks. This includes any vigorous exercise or heavy lifting.

 

 

BREAST LIFTING – WITH & WITHOUT IMPLANTS

WHY HAVE A BREAST UPLIFT?

It is not possible to recreate the fibrous support of the breasts, but they can be reshaped by removing extra skin, remodelling the breast tissue and moving the nipples up to the best position on the new breasts. Other options are to reduce the size of the areola (the darker skin around the nipple), or to have the breasts enlarged at the same time.

Having a breast uplift can make the breasts firmer and higher. After a breast uplift you may be more comfortable socially and personally. WHAT WILL HAPPEN BEFORE MY OPERATION?

You will meet with Dr. Vigo to talk about why you want surgery and what you want. Dr. Vigo will make a note of any illnesses you have or have had in the past. He will also make a record of any medication you are on, including herbal remedies and medicines that are not prescribed by your doctor.

Dr. Vigo will examine your breasts, and may take some photographs for your medical records. They will ask you if you want to have someone with you during the examination, and ask you to sign a consent form for taking, storing and using the photographs. He will measure your height and weight to make sure that it is safe to do an operation. If you are overweight, or planning to become pregnant, Dr. Vigo may suggest delaying your operation.

HOW IS THE SURGERY PERFORMED?

There are different types of uplift. The best type of uplift for you depends on your breast size and the result you want. Different types of uplift make different scars on the breasts. Whichever type of uplift you choose, the scars should not be visible when you wear normal clothing, bras and bikini tops,

The most common type of uplift, with a cut around your areola and vertically under your breast. Sometimes there is a cut in the crease under the breast to make an anchor shape. Extra skin is removed from underneath the breast, the breast is reshaped into a tighter cone and the nipples are moved to a higher level.

The size of the areola can also be reduced. If your breasts are small as well as droopy, you can have them enlarged with silicone implants. Having breast implants is the only way to significantly increase fullness above the nipples, but their weight can make the breast droop again over time.

MASTOPEXY SURGERY STEPS

Breast lift surgery involves reshaping the breast tissue and removing excess skin. Incision options include:

1) the pattern in which an incision is made around the areola

2) the vertical incision, in which incisions are made around the areola and vertically down to the breast crease

3) the inverted T or anchor incision, in which incisions are made around the areola, vertically down to the breast crease, and along the inferior breast fold.

The incision selected is based on your existing breast size and shape, the size and position of your areolae, the amount of breast sagging, and your skin quality and elasticity.

After the incisions are made, the breast tissue is lifted and reshaped, the nipple and areola are reduced and repositioned, and excess skin is removed. Sutures are placed deep within the breast tissue to support the newly shaped breasts. The incisions are then closed with dissolvable sutures.

WHAT IS THE RECOVERY STAGE LIKE?

Following surgery, you will awaken in the recovery area. A responsible adult will need to be available to escort you home. The day after surgery, you may shower while leaving the tapes on your skin intact. Patients are seen in the clinic one week following surgery. Most sutures placed will dissolve on their own. Special bandages and drains are not used. Most patients do not complain of severe pain following this surgery, but note that their breasts feel sore for several days. Pain medication is prescribed for any discomfort. Swelling will be present for the initial 3-4 weeks after surgery. The scars will continue to fade and soften for up to one year following surgery, although they will never completely disappear.

WHAT ARE DERMAL FILLERS?

Dermal fillers come in various formulations, each of which works differently in softening the lines, folds, creases and loss of volume associated with age and/or exposure to environmental aggressors. Fillers are also effective for augmenting contours of the face; to create fuller lips, a more defined jawline or plumper cheeks for example. The most popular fillers are derived from hyaluronic acid, a water-binding sugar that is already produced in the skin. Hyaluronic acid fillers are usually well tolerated and the effects, if undesirable, can often be reversed by injecting an enzyme called hyaluronidase, which helps to dissolve hyaluronic acid fillers. Other fillers are composed of synthetically derived substances, such as calcium-based microspheres. However, these synthetic options cannot be reversed and the effects last longer, which could be problematic if the results are unfavourable. More recently, fillers have been created using the patient’s own cells with results lasting up to two years.

WHO ARE DERMAL FILLERS FOR?

Fillers are appropriate for any skin type and address a multitude of age-related concerns, but only adults should have fillers, as they can inhibit bone structure if not fully developed at the time of injection. They can fill in hollows under the eyes and eliminate the depressions caused by deep creases, wrinkles, or scars, as well as plump up larger areas such as the cheeks and temples.

Recently, researchers have also discovered that injectables stimulate fibroblasts to synthesise additional collagen, so doctors are now increasingly seeing younger patients who use fillers as a preventative treatment to slow ageing. They are also popular for augmentation and contouring (fuller lips, a wider jawline, etc).

Fillers are not recommended to be used by pregnant or nursing women.

HOW DO DERMAL FILLERS WORK?

A syringe needle injects a gel-like substance into the skin at various depths. A series of injections evenly disperses the filler.
